Required Documents

Documents to gather prior to beginning the enrollment process.

The following documents are REQUIRED. OSSU MUST have all documents indicated below on file for your child to attend school; there are no exceptions.

1. Birth Documentation

One (1) of the following documents proving birth:

  • Certified copy of the student's birth certificate

  • Doctor or hospital records accompanied by a sworn statement

  • Private insurance policy

  • Baptismal records

2. Proof of Residency*

One (1) of the following documents is required (a parent/legal guardian's name and address must be shown on this document to be eligible):

  • Current tax bill
  • Current mortgage or closing statement that reflects an in-district address and parent/legal guardian's name
  • A notarized letter from the landlord stating the address of the residence being leased and the name(s) of the lessee(s), along with the landlord's address and telephone number
  • If residing with friends or family, a notarized letter from the friend/family member stating the address of the residence and the name(s) of the property owner(s), along with the property owner's address and telephone number

Two (2) of the following documents are required (a parent/legal guardian's name and address must be shown on this document to be eligible):

  • Valid Vermont driver's license with in-district address
  • Valid Vermont identification card with in-district address
  • Valid voter registration card or receipt with in-district address
  • Valid Vermont vehicle registration card
  • Current utility bill in your name
  • Valid public aid card
  • Current in-district library card with your name and address
  • Valid automobile insurance card
  • Bank statement for last or current month (financial information redacted)

*The term residency means where you actually live. Residency requirements are not met merely because one owns property in the school’s town, has a business in the town, or has a post office box in the town.

3. Student’s Immunization Record from your child’s doctor: Please provide an updated copy of your child’s immunization record. You will upload the immunization record through this online registration process, and will be contacted at a later date to submit additional required student health records.

If you need assistance obtaining a copy of your child's immunization record, please contact the school health professional at the school you child will be attending.  We can assist you in accessing this record.

It is required that you submit an immunization record even if your child is not fully immunized. A school health professional will follow up with you to discuss your child's specific immunizations if needed. If your child has not received any routine childhood immunizations, please contact the school health professional for a further discussion.
